What Services Does Hospice of Chesterfield County Provide?

Hospice of Chesterfield County provides a full spectrum of physical, emotional, psychological, social and spiritual support for terminally ill patients and their families.  Hospice allows the patient to remain at home or in other settings, such as assisted living and long-term care facilities, or even the hospital. Services Transitions: Program of Supportive Care Hospice of Chesterfield County offers the Transitions program to patients with a life limiting illness and are still receiving aggressive treatment. Transitions provides case management services, and specially trained, experienced and caring volunteers who offer support, friendship, and respite care to patients and their families while they live with their illness. Volunteer support could include assistance with errands, chores, and food preparation. The Transitions Coordinator will be glad to talk with patients, family members, caregivers, and healthcare providers about the program.
