Hospice Sabbath Planned for Nov 4-6

As you attend your place of worship the weekend of November 4 - 6, 2016 you may see a single white carnation displayed in a prominent location.  Hospice of Chesterfield County has chosen the white carnation as a symbol for the observance of Hospice Sabbath, a time when Hospice acknowledges its special relationship with the religious community.  Churches are being asked to participate by displaying a white carnation in memory of all who have received Hospice services. What Services Does Hospice of Chesterfield County Provide?

Hospice of Chesterfield County provides a full spectrum of physical, emotional, psychological, social and spiritual support for terminally ill patients and their families.  Hospice allows the patient to remain at home or in other settings, such as assisted living and long-term care facilities, or even the hospital. In addition, Hospice of Chesterfield County offers hospice aides, medical social workers, trained volunteers, and bereavement counseling.Hospice aides offer companionship and assist with bathing, personal care, light household duties, and brief periods of respite care.  Aides visit up to seven days a week, depending on the patient and the family’s needs.  Medical social workers help identify and resolve problems, coordinate resources, and provide counseling.  Our trained volunteers provide emotional support and companionship, run...

Services Transitions: Program of Supportive Care Hospice of Chesterfield County offers the Transitions program to patients with a life limiting illness and are still receiving aggressive treatment. Transitions provides case management services, and specially trained, experienced and caring volunteers who offer support, friendship, and respite care to patients and their families while they live with their illness. Volunteer support could include assistance with errands, chores, and food preparation. The Transitions Coordinator will be glad to talk with patients, family members, caregivers, and healthcare providers about the program.
