What Services Does Hospice of Chesterfield County Provide?

Hospice of Chesterfield County provides a full spectrum of physical, emotional, psychological, social and spiritual support for terminally ill patients and their families.  Hospice allows the patient to remain at home or in other settings, such as assisted living and long-term care facilities, or even the hospital. In addition, Hospice of Chesterfield County offers hospice aides, medical social workers, trained volunteers, and bereavement counseling.

Hospice aides offer companionship and assist with bathing, personal care, light household duties, and brief periods of respite care.  Aides visit up to seven days a week, depending on the patient and the family’s needs.  Medical social workers help identify and resolve problems, coordinate resources, and provide counseling.  Our trained volunteers provide emotional support and companionship, run errands, and offer brief periods of relief for the caregiver.

In addition to basic services, additional care services are provided according to the written plan of care.  This includes durable medical equipment, medical supplies, continuous care during periods of medical crisis, outpatient services, speech, occupational and physical therapy, dietary counseling and more.

If hospital care is required, the hospice team continues to work with the physician, family and the hospital staff.  Hospice of Chesterfield County is affiliated with McLeod Health Cheraw, along with many other medical facilities throughout the Carolinas.
